What Tellcrest found

A medical director emailed his billing team that he wanted them billing "at the appropriate level."

The next morning, the billing manager asked: does that mean the most expensive rate, even for 10-minute visits? She said she was "not comfortable" signing off on that.

He replied: "All of them. Don't escalate this again."

A year earlier, compliance had already flagged that 79% of visits were billed at the most expensive rate. The national average is 25%. They recommended corrective action. None was taken.

Stories don't match

"Appropriate level" on Nov 8 became "all of them" on Nov 9. Compliance flagged the problem a year earlier. Nothing was done.

Something is missing

"Don't escalate this again" means someone already tried to flag this before. That earlier complaint is not in the evidence.

The other side could argue

The first email used language that sounds like a legitimate request to code more accurately. A defense lawyer will use that.
